I think by technicality it has happened unintentionally—A pre-patched version of DA2, in Act 3, during the end of Zevran’s ‘A murder of crows’ quest, if you bring Isabella along, she can ask for some farewell sex with Zevran. Before the patch, the game would not recognize the Zevran-LI flag and he’d sleep with her without mentioning your Warden.

So yes. A LI has cheated on my Warden. :P
